UPCPMT 2014 Application Form

UPCPMT 2014 Application Form can be filled online mode only

Candidates are advised to carefully read the UPCPMT 2014 brochure before applying for the online application

First of all applicants have to fill the registration form with their mobile number as login id and date of birth as their password. Candidates also have to enter their email id during UPCPMT 2014 registration process. Candidates must note that one mobile can be used to fill only one application

A verification code will be sent to candidate after registration. Candidates can log in only after entering verification code

Candidates are required to assess their eligibility, as given in the eligibility section ofUPCPMT 2014 information brochure, themselves before applying. The examining body will not assess the eligibility. The eligibility will be checked from original documents at the time of counselling/admission and candidates found not eligible will be denied admission

Fill all the relevant and mandatory fields given in the UPCPMT 2014 Application Formas per mentioned guideline

After filling up all the mandatory field candidates must verify the entered information before clicking on submit button. Once the candidate click on submit button they cannot edit it and the information entered will be deemed as final 

Incomplete UPCPMT 2014 application form will be ejected. Applicant must not fill more than one UPCPMT 2014 application form

Candidates have to fill the downloaded UPCPMT 2014 declaration form. They have to paste the Gazetted officer or college Principal attested photograph at the designated place. Applicant also has to fill declaration form in their own handwriting and put index finger thumb impression at the designated place. They also have to take parents/guardian signature on the application. Applicants have to upload the scan declaration form and click on submit button.

After clicking on submit button candidate will receive application number on their mobile. Candidates are advised to take printout copy of their UPCPMT 2014 application form for future references.
